Minor Paperwork

Washington State RCW's direct the Washington State Department of Labor and Industries to write and enforce various WAC's in support of Washington State Workers. These laws and codes were put into place in the early 1900's to protect the rights of workers. Our concern and the focus of this document is Teen Workers.

EKCSRA is required by these codes to perform various administrative tasks.

EKCSRA has obtained the necessary endorsement to its Business License to allow it to use minor workers. Each business entity must obtain its own endorsement. This means that for each business you work for you must obtain a separate Parent/School authorization.

Parent/School Permission form

As a teen worker you need to obtain the necessary signed Permission forms from your parent/guardian and school. We have created a filled-in copy of the state form with all of the information we know about you and us. Print it; date & sign it; have your parent/guardian date & sign it; have your school data & sign it. On completion, scan it then upload (*** see Security below ) it using Youth Referee.

DO NOT FORGET TO SIGN THE FORM ON THE 4TH LINE DOWN NEXT TO THE DATE ....OR IT WILL BE REJECTED!

Proof of Age

Make a copy of your Birth Certificate or other proof of age (Drivers license or Passport) and scan then upload (*** see Security below ) using Youth Referee.

SCAN & Upload

Scan your documents to any common format (jpg, tif, pdf, etc.), then upload them to the secure file area.

Mailing Address

While the upload is preferred, the fact is that some of you will not be able to scan these documents. You can mail them to the address below; we will scan them; upload them for you; and destroy the originals. *** see Security below

Faxing the documents

While we DO NOT recommend faxing the documents because of security issues you can do so. Fax them to (206) 984-3469.

While we DO NOT recommend E-Mailing the documents because of security issues you can do so. E-mail them to accounting@ekcsra.org.

Action

When we receive the completed form and prooof of age your Profile will be modified to allow you to self-assign games. This will last until September 1st of the year executed, if you obtain only your parents signature, or September 30th of the following year, if you obtain both your parents signature and the schools signature.

*** Security

These are stored in a location that is firewalled preventing access through the Internet. We use a secure download program to display these files securely such that only you (after a secure logon) and the Administrative personal at EKCSRA have access. By law these are kept on file for 3 years.
